So the LPC is passed and the paperwork emailed in, for anyine in a similar situation, the (hopefully) complete set of forms (at least the bunch I sent in) is:

SRG1104 to convert from JAR to UK Part FCL
Colour copy of passport validated by the examiner as per the caa wording
SRG1119C to renew the rating
SRG1157 examiners report
CAA5003 to prove level 6 English proficiency
and I also sent the SRG1107 course completion certificate but I don't think that is needed.

I think you were right to send it, usually they so want the 1107 course completion certificate if they are doing the rating signature, which they are in this case.
Presumably the examiner was not UK if you had to send examiner paperwork, but if so, would that allow examiner to sign the 1199 English form? (possibly, if they were originally UK, but it would not surprise me either way).
Your expired JAA ppl was legally a UK expired FCL licence (even though live ones cannot expire) so how many forms is that to simply resurrect it with a valid rating?
