Examiners can e-mail the CAA and ask for some spare Certificate of Revalidation pages. I've got a few, Neil, if you would like me to post you one just drop me a PM. Or I can post it to your examiner directly. You'd have to get the original examiner to sign it and return it to you.

Otherwise, as you say, it's apply to the CAA for revalidation of the rating and pay the CAA fee.
