As ever, the drafting and proof-reading is sloppy.

- references to paragraphs 4 and 5 should be 5 and 6 respectively.
- the final paragraph of the explanatory notes quotes two different ORS4 numbers for the same document.

Accuracy and clarity IS important with this stuff. It's official information that is potentially safety-critical.

Reasons for errors are worth understanding; excuses less so.

Is being busy now an excuse for failing to proofread important communications and issuing it with inaccuracies that somebody will eventually have to spend time correcting?

And by doing so cleverly adding to the perceived 'busy-ness' and so providing even further justification for not doing the job they are paid for properly
