I must have led a charmed life when I flew in America. The clearance was almost always "as filed" (which in empty regions was almost always "direct"), there'd be an initial altitude, usually a cruise altitude to expect after 10 minutes, a frequency and a squawk. In the New York area, which could be a bit manic, using the preferred IFR routes from the A/FD generally kept controllers happy and clearances short.
