When I was an AME I used a positive answer to that question to consider if the accident or incident had either a possible medical cause, or medical (including psychological) consequences requiring further assessment and dealt with that as necessary.

For those that didn't have aeromedical relevance (most of them) I used to dutifully record the answers that pilots had entered in the box, but I was never certain what, if anything, the CAA did with the responses.
