If your altimeter is set up properly and set to QNH then that is indicating your altitude correctly. So you use that to keep you from exceeding the ATC clearance of not above 1500' (in the example being discussed earlier). As others have said - your transponder will show something different because it's datum is 1013hPa, ie flight levels - but that's ok for the air traffic folk because their kit knows the QNH and can convert it for them.
Unless QNH actually is 1013 your altimeter and transponder should show different altitudes.
This makes sense to me - if I've got it wrong I'm happy to be enlightened (no transponder in my aeroplane anyway so my altimeter has to be right).
