The first time that it happened was on an MEP instructional flight out of Redhill when the student presented the gear handle to the instructor and said "what do I do with this 
?". The instructor later commented that he considered continuing with the lesson but that niggling thought that you might not have any gear with which to land won out. Aerbloke's episode with the broken gear knob on the same aircraft was several years later (I assume that they didn't glue the original back in
